We structure a comprehensive plan of care based on initial and ongoing assessment of our Client’s needs at home.
- 15-minute free consultation
- Initial 1-2 hours visit and assessment
- Monthly 30-minute check-in
- Quarterly Reassessment
- Coordination of Care with Physician and others on the Client’s health team (30-minute maximum in-person or virtual session)
